HHS Marks One-Year Anniversary of Strategic Framework on Multiple Chronic Conditions

The U.S. Department of Health and Human Services (HHS) is celebrating the first anniversary of [Strategic Framework on Multiple Chronic Conditions][1], a plan to improve the overall health of individuals with multiple chronic conditions. One in four Americans - and two out of three over age 65 - have multiple chronic conditions. The Centers for Medicare & Medicaid Services' new report, _[Chronic Conditions among Medicare Beneficiaries][2]_, shows that care for its beneficiaries with multiple chronic conditions make up 93 percent of Medicare expenses.
