Survey Shows Vast Majority of Neurologists Expect To Prescribe Emerging Drug BG-12 Against Relapsing Remitting Multiple Sclerosis

As MarketWatch [1] and Decision Resources [2] have it:  "…Seventy-Four Percent of Surveyed U.S. Neurologists Indicate They Will Prescribe Biogen Idec's BG-12 for the Treatment of Relapsing-Remitting Multiple Sclerosis.." We wrote about BG-12 in a recent post [3]. Clearly, the drug had "…high efficacy and reasonable safety profile.."

How does BG-12 work? According to MedicalNewsToday (MNT) [4], "…BG-12 is the first compound that has been shown to activate a transcription pathway, which previous studies have shown defends against oxidative-stress induced neuronal death, protects the blood-brain barrier, and supports maintenance of myelin integrity in the central nervous system.."

_In other words BG-12 compound enhances the body's normal cellular protection pathways._

74% is an enormous majority. Granted, this is just a survey by Decision Resources. But, according to MarketWatch, a DowJones company, Decision Resources carries some weight.

What would happen then to the other new MS medications?

* Sanofi's Aubagio,
* Teva/Active Biotech's oral laquinimod,
* Genzyme/Sanofi/Bayer HealthCare's intravenous anti-CD52 monoclonal antibody Lemtrada.

According to MarketWatch, the news about Gilenya is not that good: "….about one-third of Gilenya prescribers and one-quarter of payers believe Gilenya's annual cost is commensurate with its clinical value…"

_U.S. Physician & Payer Forum report shows U.S. neurologists prescribing those drugs substantially less frequently than BG-12_

Interestingly, Ocrelizumab, which is also a promising drug [3], is not mentioned in the MarketWatch article.

So, the race for the second generation MS drugs is not over until the prescriptions are actually filled

_This competition is actually very good news for the new MS patient._

Braun Van Hits the Road with Challenged Athletes Foundation

Written by: Megan

[![][1]][2]

One of the youngest athletes on hand for the triathlon tries out the BraunAbility wheelchair van for the first time.

What started as a group of friends who gathered at La Jolla Cove nearly 20 years ago to support an injured triathlete has exploded into the popular San Diego Triathlon Challenge with over 7,000 competitors swimming, biking and racing to raise over $1.5 million for the [Challenged Athletes Foundation][3].

This nonprofit group, based in San Diego, exists to help fund adaptive sports equipment, training and competition expenses for individuals with physical challenges. Earlier this year [BraunAbility][4] partnered with Dodge, global partner of CAF, to donate the use of a Dodge Caravan at CAF events across the country to help transport athletes, volunteers and equipment.

With the help of a nearby BraunAbility dealership, [Ability Center][5], the 2011[ Dodge Entervan][6] was on display for athletes, volunteers and spectators to check out before and after the race…and many had the chance to explore an accessible vehicle for the very first time. Next year the Entervan turns 20 years old - and it's hard to believe that the public (especially in the disability community!) doesn't know this level of freedom exists!

Only a _very_ committed coach potato would be uninspired by the competing athletes. From the professionals who finished in record time to the exuberant little girl who zipped across on a hand bike, the mood was triumphant. Challenged athletes overcome every day.. what's a little triathlon?

_Thank you to Ability Center's Keith Miller for participating at the event and for his help with the blog and photos!_

Study Claims Immune System Boost from Specific Probiotic Bacterium Strain

We have posted earlier about the merits and challenges of deliberately introducing probiotics, or beneficial bacteria in your diet. [1]. This area of nutrition is all the rage at this time, and the food industry has brought to market many products, particularly in the form of "Greek style" yogurts, and kefirs….Other forms of probiotic supporting food can be found in the supermarketsavvy newsletter [2]

[![][1]][2]

Some Greek Style Yogurts On The Market.....(gagefit.blogspot.com)

To try to assess the health benefits, one has to turn to objective studies run in the nutrition research centers.Sure enough we can find articles from the Industry, in NutraIngredients [3]; or academic studies such as the one from Centre for Plant and Food Science, in Australia [4], which found that, quoting :

"The results of this study indicate that L. acidophilus and L. paracasei are _potential enhancers of systemic immunity and are nonpathogenic_, as suggested by their bacterial translocation profiles in healthy mice.."

_So how confident can the consumer be that there is only good in the bacteria involved in probiotics?_

There are some questions as to a shotgun approach:  Take 10 better known bacteria,  wrap them in some food product and it will do some good to somebody…Particularly people with an over/under active immune system should discuss this carefully with an MD knowledgeable in the matter, as discussed in our previous post [5], about probiotics and people with an immune system condition.

Quoting our post and ScienceDirect:

"…_**Another piece of the puzzle sounds a caution alarm: a study has incriminated Gut-Residing Bacteria Trigger Rheumatoid Arthritis, another auto-immune disease, in Genetically Susceptible Individuals…"**_

The FDA is a bit weary of broad health claims, as discussed in another article from NewHope360 [6]

Also, is boosting your autoimmune system always a good thing. Quoting [7] the U. of Maryland's warning about echinacea [an immune booster herb] :

"..People with tuberculosis, leukemia, diabetes, connective tissue disorders, multiple sclerosis, HIV or AIDS, any autoimmune diseases, or, possibly, liver disorders should not take echinacea. _There is some concern that echinacea may reduce the effectiveness of medications that suppress the immune system._ For this reason, people receiving organ transplants who must take immunosuppressant medications should avoid this herb.."

_Caution_.

**As always, talk to your Doctor**

Palliative Care and End-of-life: How it's Done

**_San Jose Care Resource Guide:  In End-of -Life decisions, it is important to realize how the "patient"  would  be_**

1. **_ Euthanized, or_**
2. **_Terminated, or_**
3. **_Killed._**

**_ Pick your choice of terms according to your belief system,  (yes, atheism is a belief system too…)_**

_**_San Jose Care Resource Guide: _**_

_Since a lot of patients are on respirators at the time this would be done. [We use the word "done"  instead of "happen" because this is an active decision on the part of the patient (if conscious), the medical palliative care  team, and of the family.]   We found this article in the  Journal of Pain and Symptom Management .  T__his study is about  the influence of morphine injection on time to death after withdrawing ventilation from a terminal patient.[** Click  Here **][1]Article is from East Carolina University._

**_**_San Jose Care Resource Guide, _** quoting:  _**_"…The mean time to death after terminal extubation was 152.7 ± 229.5 minutes without correlation with premorbid diagnoses..After extubation, each 1 mg/hour increment of morphine infused during the last hour of life was associated with a delay of death by 7.9 minutes …"__(Extubation: remaval of the respiration devices}_

_**San Jose Care Resource Guide: So,**_

* 152.7 + 229.5 minutes=382.20min  **6.37 hrs:**       We are kinder to criminals with lethal injections…see below
* 152.7 - 229.5 minutes=382.20min=-76.8 min:       Negative time a little confusing??

**Compare the duration of death by extubation we mentionned above,  to the duration of the death penalty procedure by lethal injection.  **

**Quoting** [Medscape Today][2]:   "….According to the North Carolina Department of Corrections, once the ECG monitor displays a flat line for 5 min, the warden declares death and a physician certifies that death has occurred. Execution start times and declaration times were available for 33 of the 42 lethal injections conducted in North Carolina … **Mean times to death [for lethal injection ececutions] were 9.88 ± 3.87 min for Protocol A, 13.47 ± 4.88 min for Protocol B, and 9.00 ± 3.71 min for Protocol C….."**

**San Jose Care Resource Guide: You can draw your own conclusions…**

HealthCare: California's Gov. Brown's proposed steep healthcare cuts

_**San Jose Care Resource Guide: Gov. Brown announces "painful" [for whom?] cuts in California's healthcare system. [Click Here][1]**[ ][1]**for the Kaiser Health News/California Healthline link**_

**_Quoting:_**

The cuts, which total $12.5 billion, include some large reductions in health care services, including:

* $1.6 billion cut from Medi-Cal;
* $1.5 billion savings from reducing eligibility period for state health services;
* $1 billion cut from developmental services;
* $1 billion cut from In-Home Supportive Services; and
* $ 716 million reduction in child care services, including reduction in levels of subsidies.

Ouch! Good thing it's a liberal Democrat making the cuts……
