The Department of Health and Human Services (HHS) has announced new ways for people to receive care and services in their communities instead of being admitted to a hospital or nursing home. HHS finalized the [Community First Choice rule][1], a new state plan option under Medicaid, and announced the participants in the [Independence At Home Demonstration program][2]. This program encourages health care providers to provide home-based care to chronically ill Medicare patients so they can stay in their homes while receiving medical care.
